NEW PATIENT CHECKLIST

A link to complete Patient Information Forms will be emailed to you 24 – 48 hours prior to your appointment. If you don’t receive these forms, please call (402) 496-0404.

You will need:

  • Photo identification
  • Current insurance card
  • List of current medications
  • List of allergies
  • Medical history
  • Referring physician’s notes, if applicable
  • Past MRI, CT, or X-ray scans or nerve conduction studies, if applicable
  • Copay or required down payment
  • Auto accident liability claim info, if applicable
  • Workers’ compensation claim info, if applicable
