While you can use our secure Patient Portal as a returning patient to request appointments, our portal has a number of convenient features. You can update personal information, view your medical history and statements, and make online payments.
We also offer healthcare financing with CareCredit. Whether you use your healthcare credit card to cover your deductible or to pay for treatments and procedures not covered by insurance,* CareCredit helps make the health treatments and procedures you want possible today.

NEW PATIENT CHECKLIST
A link to complete Patient Information Forms will be emailed to you 24 – 48 hours prior to your appointment. If you don’t receive these forms, please call (402) 496-0404.
You will need:
- Photo identification
- Current insurance card
- List of current medications
- List of allergies
- Medical history
- Referring physician’s notes, if applicable
- Past MRI, CT, or X-ray scans or nerve conduction studies, if applicable
- Copay or required down payment
- Auto accident liability claim info, if applicable
- Workers’ compensation claim info, if applicable
